Drafting A Living Will Workshop

Description

Five Wishes is a living will, a 12-page document that prompts individuals to consider five questions and issues: who your health-care agent should be or the person to make decisions if you’re unable to do so; what kind of medical treatment you want in various situations; addressing issues like pain medication and how comfortable you wish to be; how you want people to treat you, for example: would you prefer to die at home? and what do you want your loved ones to know about your wishes – where do you wish to be buried, for instance. This one hour workshop will cover questions like these and more to help you come to a comfortable understanding of how to handle this very important part of life. Participants will have an opportunity to complete the living will booklet during the workshop.
The booklets are available for $5.00 online at www.agingwithdignity.org. We will have a limited supply available (for $5 each) at the workshops. Please call ahead to reserve one in your name. Call 245-6585 to register.
